This toolkit offers health care providers resources to help vaccinate their patients and build or maintain a successful immunization program. This toolkit also includes information about a peer-to-peer health care provider mentoring project designed to promote vaccine administration during the well child visit for all children seen in a medical home.

CCIC has partnered with the Colorado Chapter of the American Academy of Pediatrics to create an office poster that communicates pediatric health care providers’ strong recommendations regarding childhood vaccination and their commitment to protecting patients and their families from vaccine-preventable disease. The poster is available in English and Spanish and may be customized to include your office staff photo or logo (in place of photo). All orders will be printed and shipped free of charge.
